Submitting a Claim

While on-contract, check your coverage level in your myEquity account before submitting a claim to ensure the contract has been processed and Crawford has the most up-to-date coverage information.

Claim Submission Methods

Crawford accepts Accident & Sickness benefits claims directly online, or by email or mail.

Submit your claim information, upload receipts and additional documents through the Equity-Crawford portal.

Send the completed PDF form(s) and scans/photos of supporting documents and receipts to equity@crawco.ca

Mail

Send the printed form(s) and copies of supporting documents and receipts to 100 Milverton Blvd – Suite 300 Mississauga, ON L5R 4H1

Scans and clear photos of required documents and receipts are both acceptable. Be sure to keep your original receipts for one year.

Claim Submission Checklist

Before submitting a claim, check:

Have you fully completed and signed the appropriate claim form (i.e. medical, paramedical)?
Is your Equity membership in good standing?
Have you attached an official receipt confirming the type of treatment, date of treatment, and service provider’s name/qualification? Does it include the service provider’s signature?
If you are submitting a massage therapy receipt, did you receive the treatment from a Registered Massage Therapist?
Have you kept copies of all your receipts? Crawford will not return any hard copy receipts to you.
